The goal is not simply to “trip” or have an intense experience. The goal is to use the experience as part of a structured process of healing, reflection, and change.

The most common substances people talk about in this field include psilocybin, ketamine, MDMA, LSD, ayahuasca, and mescaline. These substances are not all the same. They have different legal statuses, different effects, different risks, and different therapeutic traditions. Ketamine is legally available by prescription in many parts of the United States and is often used in clinics for depression, anxiety, PTSD, and chronic pain. Psilocybin, MDMA, and LSD remain federally controlled substances and are generally not available as standard medical treatments outside approved research or specific state-regulated programs.

That legal reality matters. When someone says “psychedelic therapy,” they may mean several different things. They may be talking about ketamine-assisted psychotherapy with a licensed clinician. They may be talking about psilocybin services in a regulated state. They may be talking about preparation and integration therapy with a therapist who does not provide the substance. They may also be talking about underground facilitation, ceremonial work, or coaching, which can vary widely in quality and legality.

At its best, psychedelic therapy is built around three parts.

The first is preparation. This is where the client and provider discuss goals, mental health history, medical risks, medications, fears, expectations, and safety. A good provider will not rush this step. Psychedelics can bring up powerful emotions, memories, and insights, and preparation helps make the experience safer and more useful.

The second is the session itself. Depending on the model, this may happen in a clinic, a licensed healing center, a therapeutic office, or another controlled setting. The provider may be a physician, therapist, facilitator, guide, or integration coach, depending on the legal structure and the type of work being done. The session may involve music, eyeshades, breathing, silence, conversation, or other support. The point is to create a setting where the person can move through the experience with care and supervision.

The third is integration. This is often the part people underestimate. Psychedelic experiences can feel profound in the moment, but the real value often comes afterward, when the person begins to understand what happened and how to bring those lessons into daily life. Integration may involve therapy, journaling, lifestyle changes, relationship work, spiritual reflection, or practical planning.

Psychedelic therapy is not magic. It is not a guaranteed cure. It is also not appropriate for everyone. People with certain psychiatric histories, heart conditions, medication interactions, or unstable life circumstances may need extra caution or may not be good candidates. Anyone considering psychedelic therapy should speak with a qualified medical or mental health professional, especially if they take antidepressants, stimulants, antipsychotics, mood stabilizers, blood pressure medication, or other prescription drugs.

The provider matters. A trustworthy psychedelic therapist or facilitator should be clear about their training, role, legal limits, screening process, fees, emergency plan, and what happens before and after the session. They should not promise a cure, pressure you into taking a substance, dismiss your medical history, or make you feel dependent on them.
The most useful way to think about psychedelic therapy is this: the medicine may open a door, but the therapeutic relationship and the integration work help you decide what to do with what you find. A good provider helps you prepare, stay safe, and make meaning. The experience is yours, but you do not have to approach it without support.

Psychedelic therapy is a broad term for therapeutic work that involves preparation, a psychedelic or psychedelic-adjacent experience, and integration afterward.
